2. Complete a free physical & upload PCP form by September 30th.

3. Save $600 on your medical premiums!

*The $600 reduction in medical premiums will be applied to 2025 plan rates which may not result in a $600 reduction compared to medical premiums to be paid in 2024.

FAQs

Already completed your physical?

If you have completed a physical since October 1st, 2023 you may already be eligible for the reduction in premiums for 2025. Please upload documentation from your physical to The Christ Hospital’s Well-Right portal (Step 2). You may upload either a completed PCP form or other documentation from your visit, such as your MyChart results.
