Plan Comparison

Fischer Homes offers a choice of three medical plans through United Medical Resources (UMR).

While all three plans provide the same services through UMR, the plans differ in how services are paid, or covered. Under the PPO Plan Associates are responsible for a co-pay at the time of service for specialist doctor visits, emergency room visits, and prescriptions. The HSA Basic and HSA Plus plans start paying for medical claims and services after Associates and their family members meet the plan deductible. Once the deductible is met, Associates pay 20% coinsurance at the time of service, versus a stated co-pay amount.

All three plans cover preventive care at $0 cost to Associates.

Pharmacy benefits for all three plans are managed by Optum Rx, through United Medical Resources.

* Our PPO plan has a separate pharmaceutical deductible for Tier 3 ($250) and Tier 4 ($500) prescriptions.

OTC At-Home Covid-19 Test

Associates can get an OTC at-home COVID-19 test at no out-of-pocket cost if you:

Purchase it an in-store pharmacy counter from one of our Preferred Retailers
Order it online through the Optum Store, which is also a Preferred Retailer
Preferred Retailer Purchase and request reimbursement

Preferred Retailers are pharmacies that United Medical Resources has made arrangements with to provide OTC at-home COVID-19 antigen tests at no out-of-pocket cost to you. Refer to the retailer’s website to verify COVID-19 tests are in stock.
